Why should I complete my Symptom Assessment?
By completing a Symptom Assessment you will help us understand how you are feeling. We can identify problems and offer help.
When do I complete my Symptom Assessment?
Complete your Symptom Assessment before each doctor's appointment at the JCC. How you feel may change every day.
Where do I complete my Symptom Assessment?
There are Symptom Assessment computer in the Maine Lobby, Clinics A through F, and in Radiation Review.
You do not need to know how to use a computer.
There are instructions at each computer to guide you.
What do I do if I need help completing my Symptom Assessment?
Step-by-step instructions are posted at each Symptom Assessment computer. A Canadian Cancer Society Volunteer may be available to help you. You may also ask a family member, friend or caregiver to help.
How long does it take to complete a Symptom Assessment?
The first time you complete a Symptom Assessment takes about 5 minutes. After the first time, it takes about 2 or 3 minutes.
Can I complete my Symptom Assessment at home?
Yes. If you have a computer and a printer, you can complete and print your Symptom Assessment before coming to your appointment at the JCC by going to this link: www.accesstoisaac.on.ca
