Help us manage your symptoms

Symptom Assessment is a set of questions about how you are feeling now and how your activity has been over the past month. By completing a Symptom Assessment you will help us understand how you are feeling.  We can identify problems and offer help. Symptom Assessment should be completed before each doctor’s appointment at the JCC as how you feel may change every day. Canadian Cancer Society Volunteers may provide assistance to anyone who needs it.
